Abstract
The World Health Organization (WHO) states that the Maternal Mortality Rate (MMR) in 2023reached 13.8 per 100,000 live births, and the Infant Mortality Rate (IMR)was 197 per 1,000 live births. The MMR in Indonesia remained high in 2023, reaching 4,482 per 100,000 live births, with an IMR of 27,530 cases. South Sumatra had a MMR of 106 in 2023 and an IMR of 666. Continuity of Care in midwifery care is a midwifery service with a continuous service model for pregnant women, childbirth, postpartum, and newborns, which aims to prevent the risk of complications during pregnancy, childbirth, postpartum, and newborns. This is expected to reduce the MMR and IMR which are still high in Indonesia. The researcher's goal is to be able to carry out Continuity of Care in midwifery care on Mrs. "S" and conduct subjective and objective data studies, assessment and plan on Mrs. "S". Continuity of Care in midwifery care for Mrs. "S" at PMB "L" Palembang from October 1, 2025 to April 17, 2025 starting from early pregnancy, childbirth, newborns, postpartum, and neonates. Data collection techniques used were primary and secondary data. The design employed SOAP notes covering data (subjective, objective, assessment, plan). The results obtained for Mrs. "S" during the first to third trimester of pregnancy did not experience complications, delivery at full term and proceeded normally, the newborn, postpartum period, and neonate proceeded normally. The researcher concludes that the Continuity of Care in midwifery care provided to Mrs. "S" during pregnancy, delivery, newborn, postpartum, and neonate took place within normal limits and Mrs. "S" was cooperative during midwifery care and was able to apply the values of Charitas’ spirituality. Suggestions for the field of practice are to maintain and improve the quality of midwifery services. Keywords: Pregnancy, Childbirth, Newborn, Neonate
