Abstract
The World Health Organization (WHO) reported that the maternal mortality rate (MMR) in 2023 reached 346 per 100,000 live births, and the neonatal mortality rate (NMR) in 2022 was approximately 6,500 newborn deaths per day. In Indonesia, the MMR was recorded at 4,482 deaths, and the NMR at 16.85 per 1,000 live births. In South Sumatra, there were 107 maternal deaths and 666 neonatal deaths. Medical records at Midwife Private Practice (PMB) “N” in Palembang from 2022 to 2024 reported zero maternal and neonatal deaths. This final report aims to provide midwifery Continuity of Care (CoC) for Mrs. “R” at Midwife “N” Indendent Practice in Palembang, during the pregnancy, childbirth, newborn, postpartum and neonates periods under the implementation of the CHARITAS values. The method employed SOAP (Subjective, Objective, Analysis, and Planning) documentation and was carried out from August 22, 2024, to March 14, 2025, encompassing pregnancy, labor, newborn, postpartum, and neonatal care at Midwife “N” Indendent Practice. Data collection involved both primary and secondary sources. The results showed that Mrs. “R” experienced normal physiological conditions during pregnancy, postpartum, and neonatal periods. However, during the first stage of labor, she was referred to a hospital with a diagnosis of premature rupture of membranes. Based on the subjective and objective data, the assessments for pregnancy, postpartum, and neonatal periods were within normal limits, while labor was managed via cesarean section at Hospital “H” due to the clinical indication of premature rupture of membranes. The care plans and implementations were aligned with the assessments.The author recommends that the clinical practice site continue to maintain the quality of midwifery services.
