Abstract
Background: Stroke is a disruption in blood flow to the brain caused by blockage or rupture of blood vessels. Consciousness is a state that includes two important aspects, namely alertness where the reticular system and brain stem are active and aware of self, other people, place and time. Assessment of the level of consciousness is the first step in establishing a diagnosis and determining appropriate intervention. The role of nurses in the ER is to assess the level of consciousness quickly and accurately in a short time in order to obtain optimal outcomes in providing comprehensive nursing care. The method for assessing the level of consciousness that is generally used is the Glasgow Coma Scale (GCS) method. Over time, several other methods were developed, one of which is the FOUR score method. Objective: This case study was conducted with the aim of applying the GCS and FOUR score methods to stroke patients. Method: The design used was a case study of three stroke patients treated in the ER. Results: FOUR score can be an alternative for assessing the level of consciousness because it can accommodate indicators in the GCS which are often difficult to assess, such as verbal assessment due to ETT installation or aphasia. Conclusion: The FOUR score is simpler to administer, easier to remember with the same score value for each component, and provides a more detailed neurological respone. Suggestion: FOUR score can be an instrument in assessing the level of consciousness in patients with verbal limitations in critical units, as an independent action by nurses.
