Abstract
The Maternal Mortality Rate (MMR) that is reported by the World Health Organization (WHO) in 2020 was 287,000 lives and the Neonatal Mortality Rate (NMR) reached 31/1000 live births. In Indonesia in 2022, the MMR was 3,572 lives and the NMR was 18,281 lives. Meanwhile, in Sumatra, the MMR was 97 lives and the NMR was 411 lives. The MMR in Palembang was 14 lives, while the NMR was 13/1000 live births. The MMR and NMR in PMB 'E' were 0 lives. One of the efforts to reduce MMR and NMR is by providing continuous obstetric care during pregnancy, childbirth, postpartum, and breastfeeding. The purpose of this final report is to provide the information of the application of midwifery continuity of care (COC) for Mrs. N in midwife Ellna Independent Practice in Palembang on pregnancy, postpartum, and neonatal periods with a spiritual and charitable approach. The method used was SOAP (Subjective, Objective, Analysis and Planning) recording, which was implemented from June 24, 2023, to January 22, 2024, covering pregnancy, childbirth, postpartum, and neonatal periods. The data collection technique used both primary and secondary data. The results obtained for Mrs. N were within the physiological range for pregnancy, childbirth, and neonatal periods. However, at 6 hours postpartum, the mother experienced hypertension. The conclusion from the subjective and objective data was that the assessment was normal for pregnancy, childbirth, newborn, and neonatal periods, but at 6 hours postpartum, a referral to Hospital "P" was made due to hypertension. The plan and implementation were carried out according to the assessment. The recommendations given by on the practice field were to apply the latest health knowledge and also to improve obstetric care based on the standard to improve maternal and child health.
