Abstract

According to data from the World Health Organization (WHO) in 2020, the global maternal mortality rate was 287,000 deaths. In Indonesia, the maternal mortality rate in 2021 was 6,856 deaths. In South Sumatra in 2021, the maternal mortality rate was 85 per 100,000 people, and in Palembang in 2019, more specific, it was 20 deaths. The infant mortality rate in Indonesia in 2021 was 10 per 1,000 live births. The SDGs aim to reduce the maternal mortality rate to 70 per 100,000 people by 2030. No cases of maternal or infant mortality were found in the PMB "E" data. As part of efforts to address maternal and infant mortality, midwifery care with Continuity of Care (COC) was provided to Mrs. "F", a 23-year-old patient at midwife "E" independent practice. COC was only provided during the patient's pregnancy and the first stage of latent phase of labor, as the patient experienced a pathological condition and was referred to a hospital. Data collection techniques were employed by using primary and secondary data. A case study method was utilized, involving SOAP records (Subjectivity, Objectivity, assessment, and Plan) from June 21, 2022, to March 13, 2023. The results revealed that Mrs. "F" had a full-term pregnancy, but during labor, there was an indication of a pathological condition after 8 hours of observation with only 1 cm of dilation, leading to a referral to the hospital. Subsequently, postpartum care, neonatal care, and puerperium care were provided, with the patient undergoing a follow-up with an obstetrician. In conclusion, a normal (physiological) pregnancy was observed, but a pathological indication was identified during labor. It was recommended that the independent practices maintained and enhanced the quality of continuous midwifery care.
