Abstract

The World Health Organization (WHO) reported that the maternal mortality rate (MMR) in the world was still high, which were 287,000 cases. In Indonesia in 2021, the MMR was 7,389 deaths and the Neonatal Mortality Rate was 20,155 deaths. One of the efforts undertaken to reduce maternal mortality was continuous midwifery care from pregnancy, childbirth, newborns, and postpartum, which aimed at the prevention to the risks of complications during pregnancy, childbirth, newborns, and postpartum. Midwifery COC to Mrs. "S" at the independent practice of Midwife "E" Palembang was carried out from July 1 2022 to January 18 2023. The method used in this case study was SOAP documentation; Subjective, Objective, Assessment, and Plans. The result showed that Mrs "S" during the first, second, and third trimesters of pregnancy was in normal condition. However, at the end of pregnancy, Mrs "S" had dry amniotic fluid. Based on the ultrasound examination, Mrs "S" was referred to the Hospital “P”. By spontaneous delivery, both the mother and the baby were in healthy condition, and the postpartum and neonates were normal. The conclusion obtained was that the pregnancy was within normal limits, but at the end of pregnancy the mother experienced dry amniotic fluid based on an ultrasound examination, so she was referred to the Hospital “P” with spontaneous labor, birth weight, postpartum, and neonate within normal limits. As the suggestions, the independent practice of midwifery should maintain and improve the quality of midwifery COC. In addition, the further researchers were to practice and improve the skills in providing midwifery COC services in the academic laboratory before the clinical practices.
