Abstract

The World Health Organization (WHO) reports that in 2019 there were 287,000 people experiencing maternal mortality with 800 cases. Additionally, it was also found out that there were 6,700 cases of Infant Mortality with the number of 2.4 million infant (23/1,000 live birth). Midwifery Continuity of Care is used to anticipate the increase of Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR). This final report aims to provide midwifery continuity of care for Mrs. “A” in the Midwife “S” Independent Practice Palembang by implementing the Charitas Spirituality approach. The treatment was given during period of pregnancy, childbirth, postpartum, and neonates from June 2, 2022 to February 6, 2023. The results were drawn descriptively by using SOAP (Subjective, Objective, Assessment, and Plan) Note method. It was found out that the condition of pregnancy, childbirth, and neonates of Mrs “A” was diagnosed to be within normal limits, whereas the labor procedure experienced stage 1 complication. In conclusion, the subjective and objective data implied that there was no complication on pregnancy, postpartum (KF2), and neonates (KN2), while the assessment was appropriately done. Furthermore, plan, implementation, and evaluation were carried out according to the needs of the mother, while the labor experience complication and should be referred. Finally the independent practice is recommended to improve the quality its midwifery continuity of care (COC).
