Abstract
According to WHO (World Health Organization) in 2020 it was found that the number of maternal deaths was still relatively high. During the process of pregnancy and childbirth, the number of mothers who died was around 287,000. One way to prevent the occurrence of MMR (maternal mortality rate) and IMR (infant mortality rate) is to provide continuous midwifery care during pregnancy, childbirth, BBL, postpartum, and family planning (KB). This final project report aims to provide midwifery continuity of care for Mrs. “T” aged 31 years at Midwife 'E' independent practice in Palembang from pregnancy, postpartum, to neonates. The method used was a case study design with SOAP recording of developments which was carried out from October 15, 2022 to April 06, 2023. The results obtained for Mrs. 'T' were normal during the first to second trimester of pregnancy, but in the third trimester of the fourth antenatal care a complication was found, namely the location of the buttocks. The patient was referred to Hospital P, vaginal delivery with partial extraction. The condition of mother and baby was healthy. The twelve-day postpartum and neonatal period was within normal limits. The conclusion obtained was that in the third trimester of pregnancy there were complications, namely abnormalities in the location of the buttocks, postpartum vaginal delivery, and neonates in healthy condition. The main suggestion for the independent practice was to improve the quality of midwifery care services on an ongoing basis (continuity of care) in accordance with the latest midwifery service standards.
