Abstract
In 2017, the World Health Organization (WHO) reported that approximately 810 women died every day due to pregnancy and childbirth complications, with most deaths caused by preventable or treatable infections and complications after pregnancy and childbirth. In Palembang, the number of maternal deaths increased from 15 in 2018 to 59 in 2020. Continuity of Care (COC) is a continuous care service for women throughout pregnancy, childbirth, postpartum, and neonatal periods, aimed at preventing complications during pregnancy, childbirth, and postpartum, and reducing maternal and infant mortality rates. This study provided COC to Mrs. "R" at the independent practice of midwife "E" in Palembang from June 22, 2022, to December 28, 2022, starting from the pregnancy, childbirth, new-born, postpartum, and neonatal periods. Primary and secondary data collection techniques were used, and a case study method was employed using the Subjective, Objective, Assessment, Plan (SOAP) note-taking method. The results showed that Mrs. "R" had a full-term pregnancy, normal delivery, normal new-born, and physiological postpartum and neonatal periods. The conclusion of this study is that COC can be used to reduce maternal and infant mortality rates. The recommendation for the practice field is to maintain and improve the quality of midwifery services.
